Hospital Discharge and Care Coordination Services

PCCS and Just Better Care – provide a hospital to home transition service aimed at reducing the likelihood of re-admission to hospital after discharge. They also provide care coordination services for patients that are otherwise at risk of hospitalisation. More information.

Primary and Community Care Services (PCCS) and Community Care Northern Beaches (CCNB) to provide a Social Work Service to Support GPs to assist in the prevention of hospital admission or re-admission for patient’s living with chronic or complex health care conditions. More information.
