Integrated Team Care (ITC) – Request for proposal of service

Sydney North Health Network is now inviting organisations to submit a Request for Proposal to provide Integrated Team Care (ITC) services for Aboriginal and Torres Strait Islander communities, with the service to commence from January 2017.

Aims of Integrated Team Care

Sydney North Health Network is looking to work collaboratively with local organisations and the community to improve Aboriginal and Torres Strait Islander health access and outcomes in Northern Sydney.

Currently, the Sydney North Health Network provides Integrated Team Care Services (ITC) across the Northern Sydney region. The aim of the program is to improve health outcomes for local Aboriginal and Torres Strait Islander people with chronic health conditions through better access to coordinated and multidisciplinary care; and to close the life expectancy gap. The intended service will need to provide improved access to culturally appropriate mainstream primary care services including general practice, allied health and specialists. Sydney North Health Network is now looking to commission this ITC service for Northern Sydney.

The establishment of the ITC has come from the Commonwealth evaluations of the CCSS (Care Coordination and Supplementary Services) and IIAMPC (Improving Indigenous Access to Mainstream Primary Care), which highlighted the benefit of having integrated Indigenous health teams. ITC formalises this team approach so that patients will be supported across the full pathway of care, from encouragement and assistance to accessing health care through to provision of multidisciplinary care.
