Social Work Service to Support GPs – Request for Proposals

Social Work Service to Support GPs – Request for Proposals

Sydney North Health Network is seeking to enhance access to and coordination of local primary health care services for people with chronic disease, who need better support around psychosocial, health, social and welfare needs.

Previous work with the HealthOne Project and the Local Health District, has identified that limitations and delays in psychosocial, social and welfare needs has an impact of health outcomes and the journeys of people through the health system.

We are hence inviting proposals for a Social Work Service to support GPs meet the specific needs of people with chronic disease, who need better support around psychosocial, health, social and welfare needs, in order to reduce the likelihood of admission or re-admission to hospital.

For further information and to access the request for proposal application form (RFP), please see the attached document. An online link to the application form will also be available on our website later this week. Submissions close 10am on 3 June 2016.

An information event on this RFP will be held on 23rd May at 4pm at the SNHN offices and via teleconference. To register for the event please email Deb Clark on dclark@snhn.org.au.

A copy of the questions and our responses will be posted on our website.
