RFP: Hospital Discharge Follow Up Service for People with Dementia

RFP: Hospital Discharge Follow Up Service for People with Dementia

REQUEST FOR PROPOSAL (RFP)

Sydney North Health Network is seeking to enhance access to and coordination of local primary health care services for people with Dementia and their carers. In consultation with our After Hours Network and Dementia Project partners, we have identified that limitations and delays in communication during transfer of care between hospital and home settings is impacting health outcomes and the journeys of people through the health system, particularly in the After Hours period.

We are hence inviting proposals for a hospital discharge follow-up service that can meet the specific needs of people with dementia and their carers.

For further information and to access the request for proposal application form (RFP), please download the word or PDF documents below. Submissions close 10am on Friday 20 May 2016.

An information event on this RFP will be held on 9th May at 4pm at the SNHN offices and via teleconference. To register for the event please email dclark@snhn.org.au.

A copy of the questions and our responses will be posted on our website.

For more information, please contact Deb Clarke on dclark@snhn.org.au.
