Sydney North Primary Health Network (SNPHN) invites local GPs, Primary Care Nurses, Pharmacists and Allied Health Professionals to this child and family health services update including support for vulnerable children/families, child protection, social issues and utilising referral pathways.

  • Dr Anna Gill, Staff Specialist Paediatrician, Head of Department of Paediatrics, Hornsby Ku-Ring-Gai Hospital
  • Vicki Laing, GP Liaison Nurse,¬†Child & Family NSLHD, with a focus on facilitating information to promote a continuum of care for families between General Practice & Child & Family Health services
  • Liz Beveridge, NSLHD OOHC Coordinator, Child and Family Health Directorate, Northern Sydney Local Health District
  • Caitlyn Cox, Social Worker, Northern Sydney Family Referral Service, based at Naremburn Family Centre
TARGET AUDIENCE:

GPs, Primary Care Nurses, Pharmacists and Allied Health Professionals

LEARNING OUTCOMES:
  • Identify reasons and frequency for 0-6 month infant presentations to ED and referral options for NSLHD Child & Family Health Services
  • Utilise information from session to identify services that vulnerable families can be referred to for support and continuity of care
  • Increase the awareness of the health assessment requirements for GPs to improve the health outcomes for children and young people in OOHC.
  • Navigate the NSLHD Out of Home Care (OOHC) Health Pathway Program and utilize new knowledge and skills to support children on this program.
COST:
There is no cost for GPs and health professionals within the Sydney North Primary Health Network region
