Australians are now living an average of 25 years longer than they were a century ago. But we are increasingly living with chronic conditions such as heart disease, diabetes, arthritis, respiratory diseases and mental illness. Furthermore, people are suffering from multiple chronic conditions, which complicate their care needs, as they need assistance from a range of medical, allied health and social services. That’s why it’s important to have a “care plan” in place that helps the care team understand the goals of treatment, from the perspective of the patient.
